Thank you for your application request for the Classical Five-Element Acupuncture Program 2018-2019.
The application forms are in the process of being updated. Applications will be emailed as soon as they are
ready on a first come, first served basis, so the sooner you leave your contact information, the closer you will
be to the top of the applicant list. Class size will be limited to ten participants. We hope for the opportunity to
share this beautiful system of body/mind/spirit medicine with you.

Refund Policy: The Program's refund policy does not apply to the purchase of books, supplies, or equipment.
All refunds will be made in accordance with the following refund schedule:
a. Applicant cancels within three business days after signing an enrollment agreement and making payment, all monies are refunded except the $150.00 application fee.
b. The Institute of Classical Five-Element Acupuncture Inc. finds it necessary to cancel the Program, its only obligation to the student will be to refund all monies paid to The Institute of Classical Five-Element Acupuncture Inc.
c. The student withdraws prior to commencing Classroom Session 2, all monies paid will be refunded within 30 days receipt of withdrawal by certified letter, less $2745.
d. No refunds will be given after the beginning of Classroom Session 2, except in the event that a schedule change by The Institute of Classical Five-Element Acupuncture Inc. forces the withdrawal of the student, in which case the unused tuition will be refunded on a pro-rata basis.
e. Any unpaid balance of tuition remains due and payable to The Institute of Classical Five- Element Acupuncture Inc. if the student withdraws or is dismissed after Classroom Session 2.
